Housing Access Peterborough Forms

The Housing Application Form is used to apply for Rent-Geared-to-Income (RGI) housing.  Housing Access Peterborough uses this form to collect the necessary information to determine eligibility and place you on the centralized waiting list.  You can fill this form out on-line, however it needs to be printed and all applicants must sign it. You can then submit this form in person, by mail, by fax, or by scanning and emailing to This email address is being protected from spambots. The Annual Update for Housing is used to update applicant information yearly.  We mail this out to applicants once a year, and it must be returned in order to maintain your spot on the waiting list. 

The Release of Information Form must be completed and submitted to Housing Access Peterborough if you give permission for us to discuss your housing situation with another person, for instance a relative, a social worker, or a shelter worker.  We will not release information about you to another person without this form being completed.

The Consent to Release Medical Information Form must be completed if you are applying for supportive Rent-Geared-to-Income (RGI) housing.  This form gives permission for your medical practitioners to share information regarding your medical needs as they relate to housing, with Housing Access Peterborough and other housing providers.  Without this form, we are unable to receive or consider any medical information that may be considered personal and confidential.

The Supportive Housing Medical Form is completed by your medical practitioner if you are applying for supportive Rent-Geared-to-Income (RGI) housing.  It is used to determine if the housing provider is able to meet your needs and provide the supports needed.  As this form will include personal information about you, it must also be accompanied by the Consent to Release Medical Information Form listed above.

The Medical Request for Additional Bedroom Form is to be completed by the applicant's medical practitioner if they are requesting an additional bedroom due to medical reasons.

The Request for Special Priority Due to Abuse Form is used to apply for Special Priority.  You can apply for Special Priority at any time, by completing and submitting this form.  For more information about this please see the How to Apply section of the website.

The Professional Declaration Form is used as part of the Request for Special Priority process and is completed by a professional who is attesting to your claim of abuse.  Once this is completed, you submit the form, and any other supporting documents, to Housing Access Peterborough.

The Agreement to Sell Form is completed by the applicant at the time of application.  Under the Housing Services Act, if you have legal or beneficial interest in any residential property in the province of Ontario, that is suitable for year round occupancy, you must agree to divest this within 180 days of the first month that you begin to receive Rent-Geared-to-Income (RGI) assistance.  This form indicates that you understand this, and agree to comply with the legislation, if you are offered subsidized housing.

The Request for Appeal Committee Review Form is used when an applicant disagrees with a decision regarding their application, that has been made by a staff member of Housing Access Peterborough.
